Your name and date of birthEnter them exactly as they appear on your statement.
Your account numberPrinted on your statement.
A credit or debit cardAlong with the billing address for that card. HSA and FSA cards work as long as they carry a Visa or Mastercard logo.
An email addressOptional. Enter it if you would like a receipt emailed to you.

When your payment posts

Payments generally appear on your account within 24 hours. Your receipt is your confirmation, so there is no need to call to verify that it went through.

Please call our billing office before you pay. We are glad to walk through the charges with you, check your insurance, or set up a payment arrangement.
